Registration

Participation is open to all persons interested in the field of dentistry. Those who wish to attend the Congress should access the online Congress Registration Form. Each registrant must fill in a separate form. 

Please note that all persons intending to attend the Congress must register, including invited Lecturers and Chairmen. All dentists MUST register as dentists. If a dentist’s spouse or partner is also a dentist, both must register separately as dentists. Overseas dentists who are members of their national dental association are eligible to pay the ADA member registration fee. Proof of membership of their national dental association is required – this must be provided to the Congress Managers at the time of registration.

Overseas dentists who are members of their national dental association are eligible to pay the ADA Member's registration fee. You must provide a copy of your National Dental Association membership with your registration.

* Students must include a copy of their student identification card.  Your student identification card must be valid at the time of the Congress. Students finishing their degree in 2008 are not entitled to the student registration rate.

** First year ADA Member Graduates must provide formal evidence of date of qualification or first registration in Australia – this must be provided to the Congress Managers at the time of registration. To be eligible for this registration rate, the date of qualification must be within a year from the date of the Congress.

To be entitled to the early registration fee you must have registered and paid by Monday 1 December 2008.

Exhibition Entry

The Exhibition will be open for industry visitors who have not registered for the Congress at a cost of A$50.00 per adult per day. Visitors wishing to attend the Exhibition will need to register and pay onsite as pre-registration is not available.

Please note: The Exhibition fee does not include access to any scientific sessions or Congress social events.

NB: Lunch is not included in the registration fee. This may be purchased from cafes within the Exhibition Pavilion.

CPD Hours

Delegates who attend the Congress will receive CPD hours for every hour they attend the scientific programme. For example, if you attend the scientific lecture programme for 7 hours then you will receive 7 CPD hours.

Payment Of Fees

NOTE:  Registrations will not be processed or confirmed until payment in full is received.

Payment for online registrations may be made by credit card only. MasterCard, Visa, Diners Club and American Express are the only credit cards accepted at the Congress. Please note all transactions by credit card will appear on your statement as payment to Conference by THPL.

Should you wish to pay by cheque, you are required to download and complete a copy of the PDF registration form. The registration form should be sent by post and must be accompanied with full payment in order for your registration to be processed and confirmed.

Cheques should be made payable to ‘ADA 2009 Congress’. All payments by cheque will incur a $A10.00 processing fee. Your name and full address should be typed or printed clearly on the back of the cheque. Cheque payments from overseas must be a bank cheque in Australian Dollars drawn on an Australian Bank. No other currency will be accepted. Please note cheques will not be accepted after 29 January 2009.

You may not pay your fees by direct transfer.

Acknowledgements

Your registration and payment will be acknowledged via email with confirmation of your requirements according to your registration form. Your registration will not be processed or confirmed if payment does not accompany your registration.

You will not receive any further publications before the Congress. Your letter of acknowledgment will include any further advice necessary prior to your arrival at the Congress.  At the Congress you will receive the final program, including speaker abstracts, and a list of delegates.

Entrance To Congress

Each member of the Congress will receive a name badge on registration.  The badge will be your official pass and must be clearly visible at all times to obtain entry to all sessions, included social functions and the Exhibition. The Congress Managers expect appropriate behaviour from attendees and reserves the right to revoke the badge and privileges of anyone behaving in an inappropriate manner.

Insurance

Registration fees do not include insurance of any kind. It is strongly recommended that at the time you register for the Congress and book your travel you take out an insurance policy of your choice. The policy should include loss of fees/deposit through cancellation of your participation in the Congress, or through cancellation of the Congress, loss of international/domestic air fares through cancellation for any reason, loss of tour monies through cancellation for any reason including airline or related services strikes within Australia, failure to utilise tours or pre booked arrangements due to airline delay, Force Majeure or any other reason, medical expenses (including sickness and accident cover), loss or damage to personal property, additional expenses and repatriation should travel arrangements have to be altered. The Congress managers cannot take any responsibility for any participant failing to arrange their own insurance. This insurance is to be purchased in your country of origin.
